San Gabriel Convalescent provides 24-hour skilled nursing care in a safe, supportive environment for residents who require ongoing medical supervision and clinical support. Our licensed nursing team works closely with physicians and interdisciplinary care staff to deliver individualized care plans focused on comfort, stability, and recovery support.
All services are provided in accordance with physician orders and facility capabilities.

Nursing & Medical Support
- 24-hour licensed nursing care
- Ongoing health monitoring and clinical assessments
- Medication administration and management
- Coordination with attending physicians and specialists
- Post-hospital care and monitoring
Clinical Care Services
- Wound care and dressing changes
- IV therapy and intravenous antibiotics (as ordered)

- Assistance with activities of daily living (ADLs)
